What happens next?

The following documentation is required for an in person meeting (if we are able to move forward after your initial inquiry, we will contact you to set a day and time for this meeting):

Copy of ALL paystubs for each person in household over age 18 for the prior two months

Copy of public assistance records stating the current monthly amount received (TANF, Social Security, SSI, Disability, etc.) for any person in household receiving such assistance

Copy of utility bills for previous two months (electric, gas, water and sewer)

Copy of ONE the following documents for the applicant and co-applicant: Birth Certificate, Social Security Card, Passport or Military ID

Social Security numbers for ALL members of the household, including children

Contact information for Landlord (phone number and address) AND prior landlord information if at current address for less than one year

Bank account information (account number, address of bank, and balance amount)

Copy of ALL bank account statements for each person in household over age 18 for the prior two months

Employment information (name of current employer(s), address, start date, and monthly wages) If less than one year with current employer, provide employment information for the last three years

Current monthly expenses (student loans, car payment, credit card payments, insurance etc.)

Copy of bankruptcy discharge (if applicable)

Copy of divorce decree or death certificate of spouse (if applicable)

Receiving Child Support (if applicable): for each child under age 18 on the application, provide a copy of the prior 12-month payment history even if payments are not being paid

Paying Child Support (if applicable): for each child under age 18 that you are currently paying child support, provide a copy of the prior 12-month payment history

Self-employed applicants and co-applicants (if applicable): must provide the past 2 years of their 1040 tax returns
